The following administrative details are needed for the above trip, please provide the confirmed participants info to me :
a)Participants particulars
b) Photocopies of passport :All particpants Must submit 2 photocopies of their passport(front page) & page indicating the date of expiry of your passport. Passport must be at least 6 months validity.
c) Indemnity Form :Please obtain permission from your parents-form duly signed.(submit hardcopy)
d) Collection of payment *:All participant need to pay not more than $150* for the trip. (* to be confirmed later)
